Office of Family and Child Services Documents

Upon hire you will need to fill out these forms so we can send them to the Office of Child and Family Services.

OCFS 6001

Household Information — This is where you will need to input the information for all the members of your household.

OCFS 6004

Medical Statement — This form will need to be printed out and filled out by your primary physician. Call the center if you need a printed out copy.

OCFS 4930

Fingerprints — This form is used for fingerprinting. You will need this form at your fingerprint appointment. Please print it out or call the center for a copy beforehand.

OCFS 6002

Qualifications — This form will be filled out according to the qualifications you already possess such as degrees or a CDA. It will also ask for experience with children.

OCFS 6005

Criminal Conviction Statement — This form is used to ensure you have not committed any felonious crimes.

IDENTOGO

This is the website used for fingerprinting. You can make the appointment yourself or we can do it for you. There are multiple locations in the area. Take a look at the website here and if you would like us to set it up for you, please call us.

References — This form will be where you enter in information for references.

OCFS LLDS 3370

Statewide Central Database Check — In this form you will enter in all members of the household and their addresses within the last 7 years. This is to check for child abuse convictions or related crimes.
